What it is

Actions grouped into 30, 60 and 90 day bands, derived from the failed controls, the domain scores, the cost position and the evidence gaps.

Why banded by time

An undifferentiated list of forty actions is not a plan. Bands force a sequence, and a sequence is what management can be held to at the next meeting.

Why the bands are capped

Because the matrix is a table a board reads. Each band carries a bounded number of items.

What happens to what does not fit

It is stated. The pack reports how many further actions exist in each band rather than silently truncating, so a reader knows the matrix is a selection and how large a one.

Silent truncation would read as completeness.

When a band is empty

The pack says so, with the observation that produced that conclusion, rather than leaving a blank that could be read as an omission.
